Srishti School of Art, Design and Technology ( Srishti ) PhD Entrance Examination Application Procedure – 2015

Completion and submission of application form:

Please download this PhD Program Application Form and fill up all the relevant sections.

Please type or print legibly in black ink.

Candidates may submit only one Application Form per year.

The completed PhD Program Application Form must reach Srishti office no later than March 31, 2015.

Please note that material submitted with this application will be retained by Srishti.

Application checklist:

A Demand Draft favouring “Srishti” for Rs. 5,000/- ( non-refundable Application Fee ), payable at Bangalore, with your name clearly written on the reverse r Reference forms fully completed and signed by the referee ( two references required in sealed envelopes )

Copies of your most relevant mark sheets / transcripts / degree certificates.

A copy of your birth certificate

Details of visa ( if you are not of Indian origin )

Two recent colour passport size photos

Additional sheets used for your Statement of Research Interest

Additional sheets used for describing relevant skills and research / work experience

Curriculum Vitae

Writing sample such as a paper or publication

Portfolio of creative works if relevant
