Issue of Information Brochure and Application Form:

The prospectus containing Application Form for entrance examination for admission for the academic year 2015-16 can be obtained from below Address

The Application Form and Prospectus can be obtained from above mentioned address during working hours in person by submitting the demand draft of Rs.3,000/- drawn in favor of NIMS University, Jaipur payable at Jaipur. If the form is required by post, send a D.D. of Rs.3,200/-.

The last date of submission of duly filled in Application Form by the candidate is January 21, 2015 ( 5:00 PM ).

Application Form can also be downloaded from the Official website and send the filled-in form along with requisite fee on the above mentioned address.

Candidate shall fill-up the Application form in his / her own handwriting in capital letters ( block letters ) written neatly and legibly.

The completed Application Form along with necessary enclosures / documents must be sent by Speed Post or Registered Post or by hand to the address given above.

For sending Admit Card the Applicant should send the self addressed envelope ( provided along with Application Form ) and paste a postage stamp on it equivalent to the cost of Speed Post.

The same envelop will be used for sending the application form, however, University will not be responsible for the loss of postage during transition.

Filling of Application Form:

A candidate seeking registration to the AIPPGMDEE-2015 entrance examination is required to send his / her application in the prescribed Application Form only, appended in this Information Brochure.

A candidate shall not fill and submit more than one Application Form for the entrance examination.

Before filling up the Application Form the candidate must familiarize himself / herself with the information given in this brochure and shall satisfy his / her eligibility to appear in the entrance examination.

All the candidates are required to go through the Information Brochure very carefully and acquaint himself / herself with all the requirements in regard of the filling up the Application Form.

Fill the form in English using capital letters, ( except for signature ).

Any discrepancy in the statement and / or submission of incomplete forms may lead to rejection of the application / cancellation of admission.

The candidate is required to paste a most recent color photograph ( not older than three months ) at the appropriate place on the application and in the hall ticket / admit card.

The photograph must be signed by the candidate at the edge and should not be defaced.

The candidate shall write his / her address correctly on the envelope.

Documents required at the time of submission of Application Form:

Photo copy of mark-sheet of final professional MBBS / BDS Examination

Those who have already completed their compulsory Rotatory Internship must submit a photocopy of completion certificate.

Those who are undergoing Rotatory Internship must submit a certificate from the Dean or Competent Authority indicating probable date of completion of Internship

Photocopy of permanent / provisional registration from MCI / DCI or State Medical Council / State Dental Council

All these should be attested by a Gazette Officer and also self attested
