Get the application for from your nearest designated center: You can get the CHSL Application form, from the Local or near by Photostat shops or you can self print it from the Official PDF Document containing the SSC CHSL 2017 Notification.

2) Examination Center and Center code: These details are given in the Official Notification Itself.

5) You have to paste your recent photograph in the space provided.
6) And Put your signature at the specified place in the Application form.

You have to fill the application and submit this application form to the concerned regional/sub-regional office of the SSC.
